The information will be collected via the website contact form, email, or third-party reservation systems (such as our electronic medical record - EMR). Additionally, we may collect information via email and voicemail transcripts, depending on how you choose to contact us. All information is gathered only after you explicitly provide said information and it is used to provide you with requested services and support.

If you prefer to opt out of text messages, reply with STOP to any of our texts. Once you opt out, we will promptly remove you from our communication list, and you will no longer receive reminder/appointment messages from us.
